If the person will change school stage or turn 18 before the assessment—for example, moving from year 6 to year 7—please contact me before booking so that I can advise which option to select and provide the correct questionnaire.
For a child at primary school, up to and including year 6
For a young person currently in years 7–11
For a young person aged 16 or 17 attending sixth form or college
For an 18-year-old attending sixth form or college
For an adult who is no longer attending sixth form or college
